2023 Community Transformation Program

Request for Proposals


Wellstar Health System is accepting proposals from community partners focused on the use of technology to increase access to health care and/or increase access to social determinants of health support services for communities experiencing health disparities. 


Submission Steps

Proposals are being requested through a two-step process, beginning with a Notice of Intent (NOI) to describe your organization and its intention to submit a full proposal.

Step 1 Notice of Intent ­Due October 13, 2023: NOIs will be reviewed for fidelity and a select number of partners will be invited to submit a full proposal. NOIs (one per non-profit, community-based organization) must be submitted electronically to Survey Monkey Apply by 11:59pm EDT, Friday, October 13, 2023.

Step 2 Full Proposal Due November 3, 2023: Each proposal is carefully considered, and we encourage all partners to review this Request for Proposals in full. Priority will be given to partners who:

  • Focus their initiative on target populations experiencing health disparities as identified in Wellstar’s 2022 Community Health Needs Assessment (https://www.wellstar.org/about-us/pages/community-health-needs-assessment.aspx).
  • Directly address sustainable technology solutions that increase organizational capacity to address access to care and social determinants of health support challenges within communities, even after Wellstar support ends.
    • Consider these questions: 
      • What is the most important tech platform your organization uses today?
      • What kind of technology are you most focused on implementing in the next 12 months to help you scale?

Eligibility

Wellstar Health System will partner with a selection of non-profit, community-based agencies with demonstrated experience in improving and measuring health outcomes. Ineligible organizations include: For-profit organizations; Individuals; Individual schools, excluding institutes of higher education; International causes or campaigns and Political organizations.


About Leveraging Technology

Wellstar will support initiatives that leverage technology to produce long-term, sustainable changes in communities we serve. Initiatives should be tailored to a community need and align with each partner’s mission. At the end of 12 months, partners are expected to demonstrate improvements in:

  • Access to Health Care so that residents receive timely, high-quality, and culturally-appropriate medical care when they need it; or,
  • Access to Social Determinants of Health Support Services so that residents receive timely, high-quality food, employment, housing, transportation, education, and legal resources to improve daily living when they need it.


Examples of sustainable technology solutions applied to health care and social determinants of health support improvement initiatives include, but are not limited to:

  • Hardware for live language access and sign language interpretation services in community clinics
  • Air quality sensors and medical care linkage system for residents in and around neighborhoods with high rates of asthma hospitalizations
  • Development of a health risk factor assessment and care referral app
  • Cloud server space to house a social support referral technology platform for homeless, LGBTQIA youth
  • Cameras to facilitate web-based behavioral health appointments in schools with children at-risk for suicide
  • Virtual reality headgear to facilitate alternative pain management solutions for community residents at-risk for opioid abuse
  • Installation of telehealth technology in a non-clinical, faith-based location
  • Purchase of image recognition technology to assess client meals, captured via a smartphone camera, for nutritional value and follow-up referrals to counseling and food sources

Informational Webinar: September 25, 2023

Notice of Intent Deadline: October 13, 2023

Partners notified of their invitation to submit a full proposal: October 20, 2023

(Upon Request) Full Proposal Deadline: November 3, 2023

Announcement of Award: December 1, 2023 (during Community Transformation Program Summit, time & location TBA)

Project Period Start: December 4, 2023

Check disbursement upon successful execution of memorandum of understanding between Wellstar and Grantee

Notice of Intent Review

A Notice of Intent (NOI) will meet the following criteria:

  • Describe the initiative (i.e. leveraging technology to improve community health)
  • Indicate partners who will share accountability for initiative outcomes
  • Share SMART objectives that will inform initiative success
  • Include a funding request of no more than $10,000 (US Dollars)

NOIs (one per non-profit, community-based organization) must be submitted electronically to wellstar.smapply.io by 11:59pm EDT, Friday, October 13, 2023. Partners must visit the web address to create an account and view the NOI submission template before drafting a response.

Scoring Criteria

NOIs will be reviewed by Wellstar Health System based on fulfillment of NOI criteria.

Full Proposal Review

A proposal will meet the following criteria:

  • Indicate a targeted population experiencing health disparities as identified in Wellstar’s 2022 Community Health Needs Assessment
  • Provide evidence of past success improving and measuring health outcomes
  • Detail initiative activities and timelines
  • Evidence partners who will share accountability for initiative outcomes with letters of support
  • Share details about how initiative progress will be tracked
  • Include an itemized budget
  • Complete all required Wellstar declarations

Any omissions will automatically deem a proposal ineligible.

Proposals must be submitted electronically by 11:59pm EDT, November 3, 2023. 

Scoring Criteria

Proposals will be reviewed by Wellstar Health System based on fulfillment of criteria using a defined rubric.
