Pilot Initiative: Operation M.I.S.T
In the 2019 CHNA, Wellstar Health System recognized the maternal mortality and morbidity crisis that is impacting birth givers and the health of their babies. In response, the Wellstar Community Health (WCH) team seeks to establish a new partnership with Operation M.I.S.T (Monitor, Intervene, Survive, Thrive).
Submission Steps
Proposals are being requested through a two-step process, beginning with a Notice of Intent (NOI) to describe your organization and its intention to submit a full proposal.
Step 1 Notice of Intent Due May 20, 2021: NOIs will be reviewed for fidelity and a select number of partners will be invited to submit a full proposal. NOIs (one per non-profit, community-based organization) must be submitted electronically to NOI Submission by 11:59pm EDT, Tuesday, May 20, 2022.
Step 2 Full Proposal Due August 20,2022
About Leveraging Technology
Wellstar will support initiatives that leverage technology to produce long-term, sustainable changes in communities we serve. Initiatives should be tailored to a community need and align with each partner’s mission. At the end of 12 months, partners are expected to demonstrate improvements in:
Access to Health Care so that residents receive timely, high-quality, and culturally-appropriate medical care when they need it; or,
Access to Social Determinants of Health Support Services so that residents receive timely, high-quality food, employment, housing, transportation, education, and legal resources to improve daily living when they need it.

Notice of Intent Review
A Notice of Intent (NOI) will meet the following criteria:
Describe the initiative (i.e. leveraging technology to improve community health)
Indicate partners who will share accountability for initiative outcomes
Share SMART objectives that will inform initiative success
NOIs (one per non-profit, community-based organization) must be submitted electronically to NOI Submission by 11:59pm EDT, Friday, May 27, 2022. Partners must visit the web address to create an account and view the NOI submission template before drafting a response.
Full Proposal
A proposal will meet the following criteria:
Indicate a targeted population experiencing health disparities as identified in Wellstar’s 2019 Community Health Needs Assessment
Provide evidence of past success improving and measuring health outcomes
Detail initiative activities and timelines
Evidence partners who will share accountability for initiative outcomes with letters of support
Share details about how initiative progress will be tracked
Include an itemized budget
Complete all required Wellstar declarations
Any omissions will automatically deem a proposal ineligible.
Proposals must be submitted electronically by 11:59pm EDT, May 27, 2022.
